The forms registry is a critical component of Microsoft Outlook Web Access. If this error is not addressed, users might not be able to access the Outlook Web Access client. The Help and Support Center provides information about troubleshooting the forms registry.
Event InformationAccording to Microsoft :

Cause :

This Error event indicates that Microsoft Office Outlook Web Access did not initialize because the forms registry folder referenced in the event description does not exist. The forms registry folder contains files that you must have to display an item or folder referenced in an Outlook Web Access HTTP request. If the required forms registry folders or files do not exist, users cannot access their mailbox using Outlook Web Access.

Resolution:

To resolve this error, replace the forms registry folder with the default folder on the Microsoft Exchange Server 2007 DVD. Follow these steps:
  1. Locate the forms registry folder in the Setup\ServerRoles\ClientAccess\owa\formsfolder on the Exchange 2007 DVD.
  2. Copy the contents of the formsfolder to theC:\Program Files\Microsoft\Exchange Server\ClientAccess\Owa\formsfolder on the computer that is running the Client Access server role, whereC:\is the directory to which you installed Exchange 2007.
